The First Year Together

Discussion Questions

  1. This case does not provide detailed information how the First Year Together transition project was introduced to Carolyn and her husband. If you were in the position of offering the project's services to this family, how would you do it? How would you contact the parents? Where would you speak to them? Would you offer written information? What words would you use in explaining the project and offering services.

  2. The first visit to a family's home is often uncomfortable for both the parent(s) and the professional. In this case, what did Bonnie do or say to reduce the tension and begin establishing rapport? What are some other strategies that you might use with families to establish rapport in such first encounters?

  3. How well do you think Bonnie addressed the issue of Carolyn's continued pumping of breast milk for Evan? Would you have handled it differently?

  4. Evidently Carolyn felt a need to talk about the events and the feelings she experienced at Evan's birth and during his stay in the intensive care nursery. Was it appropriate for Bonnie to serve as her sounding board in these maters? Would it have been appropriate for Bonnie to suggest that Carolyn seek professional counseling?

  5. Did Bonnie provide an appropriate level of support in finding child care for Evan so Carolyn could resume some of her own activities? Was Bonnie too directive or not directive enough?

  6. List at least five specific things that Bonnie did or said that exemplify the concepts of "enablement" or "empowerment."

  7. Suppose you want to initiate a neonatal intensive care unit (NICU) to home transition project in your own community. Write a brief proposal to submit to the town council or city hall. Your proposal should contain the following:

    • A statement of the need for the project (i.e., rationale)

    • A list of the proposed outcomes (i.e., objectives)

    • An overview of the strategies to be used by the project staff (i.e., method)

    • A statement of how the project will fit into the existing service delivery system (i.e. interagency coordination)

  8. Suppose you are responsible for training and supervising staff for a project similar to First Years Together. In this position, one of your tasks is to develop a 3- to 5-page paper entitled, Guidelines for Home Visitors." It will be used to introduce staff to the project's philosophy and some of the basic principles and strategies for conducting home visits.
